Trip Meeting

On Thursday, January 14th at 7:00 pm, we will have another Japan meeting in the band room. It is important that I have 2 copies of your passport and a “Minor Consent” form for each minor not traveling with both parents. At this meeting we will discuss current rooming arrangements, more etiquette, and view a video on some things to expect. Also, unless you have made other arrangements with me, all payments are due by January 15th. I am really getting excited about this trip and look forward to this meeting.

Minor Consent to Travel Form

Below is the link for the Minor Consent to travel form. All students traveling without both parents must complete this form and submit to Mr. Goff.
